Instructions: Please answer the following questions to the best of your ability. This questionnaire is confidential. The questions, answers, or any information related to the contents of the questionnaire are not to be discussed with anyone besides Tribunal staff. In its search for the truth, the Tribunal is interested in knowing what you know. If you do not know an answer, please state this in your response. If you have any questions about the questionnaire, please contact the Lead Judge who sent you the Tribunal letter.
